Criteria for eligibility:

·         Patient of record of Dr. Rebecca Pikos or Dr. Clayton McEntire

·         Graduating high school senior with a letter of acceptance from an educational institution

·         A GPA of at least a 3.0

·         Community service hours

Application Process:

Please submit ALL the following documents before scholarship deadline to info@shinyhappysmiles.com or if preferred in office during normal business hours. Applications will be considered incomplete if missing any of the following:

1.      Completed application form

2.      Two (2) letters of recommendation

3.      Personal essay

4.      Letter of acceptance from an educational institution

5.      Proof of GPA

6.      Proof of community service hours

Essay Topic:

            What are you planning to study and how you got interested in that degree OR how you plan to make a difference in the world using your degree

Deadline: June 7, 2024

Selection process:

            Committee consisting of Dr. Rebecca Pikos and Dr. Clayton McEntire will select recipient. Winner will be notified no later than June 21, 2024.
